Frequently Asked Questions

What do I need to do to prepare for the appointment?

Once your appointment is confirmed, you will receive an email with all the details. Specifically, I will request a record of your child’s diet for three days so I can understand your child’s typical intake, habits, and routines. I also request your child’s growth chart from their pediatrician. I will also send an insurance form and intake questionnaire for you to fill out prior to your appointment.

Do you accept insurance?

Yes, I am in-network with Blue Cross Blue Shield, Cigna, and United Healthcare.

I will always submit a claim for your visit to insurance first and bill you later if necessary.

How do I know if my insurance covers nutrition services?

Your coverage depends on your individual plan. It’s always a good idea to check with your insurance provider before your appointment. You can ask if you have coverage for the following:

  • Procedure code (aka CPT code):
    • 97802 (initial visit)
    • 97803 (follow-up visit)
  • ICD-10 code (aka diagnosis code): Z71.3 (nutrition counseling and surveillance). Additional diagnosis codes may be used

What is the cost of the appointment without insurance coverage?

The initial appointment is $120 and any follow-up appointment is $25/15 minutes with a minimum of 30 minutes.

Can I use my Health Spending and Flex Spending debit card to pay?

Yes, absolutely!

How long do appointments last?

Initial appointments are usually 60 minutes. Follow-up appointments can last between 30-60 minutes.

Do you communicate with our pediatrician and other specialists involved in our child's care?

Absolutely! Communication with your healthcare providers and continuity of care is of the utmost importance to us!

Client Testimonals

At 6months my son was diagnosed with FPIES, a gastrointestinal allergy, which causes extreme vomiting and can lead to shock. When I first saw Leslie I was a mess. I was scared to give my son anything to eat… Leslie helped make something that is not only scary, but life threatening, a lot easier for my entire family to cope with and be educated on. Leslie helped my little boy thrive! My son now 15m eats and loves to eat! He is healthy and happy! We are aware of his food triggers and avoid them while still allowing him to eat things everyone else gets to enjoy!
